ST:STUDY_SUMMARY                 	Friedreich’s Ataxia (FRDA) is an autosomal neurodegenerative disease caused by
ST:STUDY_SUMMARY                 	the deficiency of protein frataxin. Frataxin functions in the assembly of
ST:STUDY_SUMMARY                 	iron-sulfur clusters that are important for iron homeostasis and metabolic
ST:STUDY_SUMMARY                 	functions. To identify metabolic features that can be used for potential
ST:STUDY_SUMMARY                 	biomarkers in FRDA plasma, we performed a targeted multi-omics (metabolomics,
ST:STUDY_SUMMARY                 	lipidomics, and proteomics) analysis using discovery-validation cohort design.
ST:STUDY_SUMMARY                 	Muti-omics analysis revealed that FRDA patients had dysregulated sphingolipid
ST:STUDY_SUMMARY                 	metabolism, phospholipid metabolism, citric acid cycle, amino acid metabolism,
ST:STUDY_SUMMARY                 	and apolipoprotein metabolism. Sphingolipid dysfunctions were revealed by
ST:STUDY_SUMMARY                 	decreased very long chain ceramides but unchanged long chain ceramides in FRDA
ST:STUDY_SUMMARY                 	plasma, which resulted in the increased ratio of long chain ceramides to very
ST:STUDY_SUMMARY                 	long chain ceramides. Decreased very long chain ceramides distinguished FRDA
ST:STUDY_SUMMARY                 	patients from healthy controls and showed good predictive capacities with AUC
ST:STUDY_SUMMARY                 	values from 0.75 to 0.85. Furthermore, by performing lipidomic and stable
ST:STUDY_SUMMARY                 	isotope tracing experiment in induced pluripotent stem cell differentiated
ST:STUDY_SUMMARY                 	cardiomyocytes (iPSC-CMs, we demonstrated that frataxin deficiency affected
ST:STUDY_SUMMARY                 	ceramide synthase (CerS2), and preferentially enriched long chain ceramides and
ST:STUDY_SUMMARY                 	depleted very long chain ceramides. Moreover, ceramide metabolism was
ST:STUDY_SUMMARY                 	differentially regulated in a tissue-specific manner. Finally, machine learning
ST:STUDY_SUMMARY                 	model increased the prediction of FRDA using the combination of three
ST:STUDY_SUMMARY                 	metabolites (AUC > 0.9). In conclusion, decreased very long chain ceramides are
ST:STUDY_SUMMARY                 	potential biomarkers and therapeutic target in FRDA patients.
