Recurrences ST:STUDY_SUMMARY are attributable to clinically undetectable endocrine-tolerant persister cancer ST:STUDY_SUMMARY cells that retain tumor-forming potential. We observed that persistence occurred ST:STUDY_SUMMARY stochastically without genetic predisposition. Genome-wide screening in ST:STUDY_SUMMARY persisters revealed a survival mechanism involving metabolic reprogramming with ST:STUDY_SUMMARY reliance upon mitochondrial respiration. Proteomic profiling showed reduced ST:STUDY_SUMMARY levels of glycolytic proteins in persisters. Metabolic tracing of glucose ST:STUDY_SUMMARY revealed an energy-depleted state in persisters where oxidative phosphorylation ST:STUDY_SUMMARY was required to generate ATP. Persisters exhibiting residual proliferation in ST:STUDY_SUMMARY human breast tumors following neoadjuvant endocrine therapy showed increased ST:STUDY_SUMMARY mitochondrial content. Pharmacological inhibition of oxidative phosphorylation ST:STUDY_SUMMARY suppressed the tumor-forming potential of persisters and synergized with the ST:STUDY_SUMMARY anti-estrogen fulvestrant to induce regression of patient-derived xenografts, ST:STUDY_SUMMARY supporting therapeutic targeting of mitochondrial metabolism to help eradicate ST:STUDY_SUMMARY residual disease.
