PR:PROJECT_SUMMARY               	linking collagen IV defects to podocyte injury remain incompletely understood.
PR:PROJECT_SUMMARY               	Emerging evidence suggests that lipotoxicity and lipid dysregulation may play a
PR:PROJECT_SUMMARY               	pivotal role in mediating podocyte damage in AS, akin to its established role in
PR:PROJECT_SUMMARY               	diabetic kidney disease (DKD). We sought to identify plasma and urine lipid
PR:PROJECT_SUMMARY               	alterations in autosomal dominant and X-linked AS compared with DKD and healthy
PR:PROJECT_SUMMARY               	controls. Using liquid chromatography coupled to mass spectrometry (LC-MS), we
PR:PROJECT_SUMMARY               	annotated 580 and 203 lipid species in plasma and urine, respectively. Volcano
PR:PROJECT_SUMMARY               	plot and ROC analyses (AUC ≥ 0.80) identified key lipids, including urinary
PR:PROJECT_SUMMARY               	HexCer 18:0(3O)/24:0(2OH) and CAR 12:0. These analyses highlighted the most
PR:PROJECT_SUMMARY               	relevant lipotoxic pathways, which may warrant deeper investigation for drug
PR:PROJECT_SUMMARY               	development in AS. Compared to controls, AS exhibited unbalanced sphingolipid
PR:PROJECT_SUMMARY               	catabolism, ceramide overload, and impaired fatty acid β-oxidation, alongside
PR:PROJECT_SUMMARY               	phospholipid and cholesterol imbalances suggestive of compromised ABCA1-mediated
PR:PROJECT_SUMMARY               	lipid efflux and mitochondrial dysfunction. Comparisons with DKD indicated a
PR:PROJECT_SUMMARY               	shared lipotoxic environment with ceramide elevation and disrupted fatty acid
PR:PROJECT_SUMMARY               	metabolism. However, disease-specific adaptations emerged, with severe ABCA1
PR:PROJECT_SUMMARY               	dysfunction and marked phospholipid/cholesterol derangements in DKD, whereas AS
PR:PROJECT_SUMMARY               	showed pronounced sphingomyelin depletion. These findings demonstrate that AS
PR:PROJECT_SUMMARY               	involves distinct lipidomic disruptions and underscore shared lipotoxic
PR:PROJECT_SUMMARY               	mechanisms. This improved understanding of disease-specific lipid imbalances

CO:COLLECTION_SUMMARY            	We collected urine and plasma samples from 63 case subjects with a diagnosis of
CO:COLLECTION_SUMMARY            	ADAS (n=51) or XLAS (n=12). The inclusion criteria were a genetic analysis with
CO:COLLECTION_SUMMARY            	pathogenic or likely pathogenic variants in COL4A3-5 with the presence of
CO:COLLECTION_SUMMARY            	hematuria. The same samples were collected from 15 patients with biopsy-proven
CO:COLLECTION_SUMMARY            	diabetic kidney disease (DKD) and from 20 healthy volunteers, who served as
CO:COLLECTION_SUMMARY            	control groups. Clinical and demographic data were collected from the medical
CO:COLLECTION_SUMMARY            	records. The study was conducted in accordance with the principles outlined in
CO:COLLECTION_SUMMARY            	the Declaration of Helsinki with approval from the ethics committee of Hospital
CO:COLLECTION_SUMMARY            	Universitario 12 de Octubre. Written consent was obtained from patients at the

#TREATMENT
TR:TREATMENT_SUMMARY             	PLASMA SAMPLES: Plasma samples underwent deproteinization and lipid extraction
TR:TREATMENT_SUMMARY             	using an all-in-one single extraction method employing a solvent mixture
TR:TREATMENT_SUMMARY             	composed of MeOH/MTBE/CHCl3 (4:3:3, v/v/v) for the isolation of non-polar
TR:TREATMENT_SUMMARY             	lipids(1). The lipid extraction method started with thawing the samples on ice
TR:TREATMENT_SUMMARY             	followed by a 2 min vortex homogenization. Subsequently, 40 µL of the plasma
TR:TREATMENT_SUMMARY             	sample was mixed with 800 µL of the solvent mixture – also containing the
TR:TREATMENT_SUMMARY             	internal standards (IS): 1 ppm C17-sphinganine and 2 ppm d31-palmitic acid –.
TR:TREATMENT_SUMMARY             	The resulting mixture was vortex-mixed for 20 min followed by sample
TR:TREATMENT_SUMMARY             	centrifugation at 16,100 x g for 10 min at 15 °C. Then, 300 µL of the
TR:TREATMENT_SUMMARY             	supernatant were transferred to LC Chromacol (Thermo Fisher Scientific, Madrid,
TR:TREATMENT_SUMMARY             	Sain) vials with insert and centrifuged at 16,100 x g for 10 min at 15 °C prior
TR:TREATMENT_SUMMARY             	to the analysis. Blank solutions were prepared containing 40 µL of H2O and 800
TR:TREATMENT_SUMMARY             	µL of the solvent mixture and same procedure was followed. For quality control
TR:TREATMENT_SUMMARY             	(QC) samples, three independent pools were prepared by pipetting 10 µL of each
TR:TREATMENT_SUMMARY             	sample – for QCTOTAL –, 10 µL of Alport samples (cases 1 and 2) – for
TR:TREATMENT_SUMMARY             	QCCASES –, and 10 µL of control samples (DM patients and healthy individuals)
TR:TREATMENT_SUMMARY             	– for QCCONTROL–. Then 40 µL of each of the pools were transferred to
TR:TREATMENT_SUMMARY             	Eppendorf™ tubes and 800 µL of the solvent mixture were added. The same
TR:TREATMENT_SUMMARY             	procedure as in sample preparation was followed. Both blank and QC samples were
TR:TREATMENT_SUMMARY             	processed in parallel with the rest of plasma samples. URINE SAMPLES: Urine
TR:TREATMENT_SUMMARY             	samples were prepared at the "Centro de Metabolómica y Bioanálisis, CEMBIO"
TR:TREATMENT_SUMMARY             	(Madrid, Spain), following a monophasic extraction method (MeOH:EtOH (1:1,
TR:TREATMENT_SUMMARY             	v/v)), tested and developed in our laboratory to obtain the urine lipidomics
TR:TREATMENT_SUMMARY             	fingerprint. Briefly, the samples were thawed on ice and then vortex-mixed for 2
TR:TREATMENT_SUMMARY             	min. On ice, 100 µL of urine sample was mixed with 300 µL of a cold (-20 °C)
TR:TREATMENT_SUMMARY             	mixture of MeOH:EtOH (1:1, v/v), and a mixture of nonendogenous internal
TR:TREATMENT_SUMMARY             	standards (C17-Sphinganine at 1 ppm and d31-palmitic acid at 2 ppm). Samples
TR:TREATMENT_SUMMARY             	were vortex-mixed for 30 minutes. After centrifugation for 10 min at 16.1 rpm at
TR:TREATMENT_SUMMARY             	15 °C, 100 µL of the supernatant was collected and transferred into an HPLC-MS
TR:TREATMENT_SUMMARY             	vial with a glass insert. Finally, all the vials were centrifuged at 2000g at 15
TR:TREATMENT_SUMMARY             	°C for 10 min before injecting into the system.

CH:CHROMATOGRAPHY_SUMMARY        	The Agilent 1290 Infinity II Multisampler system was employed with a multiwash
CH:CHROMATOGRAPHY_SUMMARY        	option, and 1 µL of extracted samples was injected. The multisampler
CH:CHROMATOGRAPHY_SUMMARY        	temperature was maintained at 15 °C to ensure the stability of compounds and
CH:CHROMATOGRAPHY_SUMMARY        	prevent lipid precipitation. For chromatographic separation, an Agilent
CH:CHROMATOGRAPHY_SUMMARY        	InfinityLab Poroshell 120 ECC18 (3.0 × 100 mm, 2.7 µm) (Agilent Technologies)
CH:CHROMATOGRAPHY_SUMMARY        	column and a compatible guard column (Agilent InfinityLab Poroshell 120 ECC18,
CH:CHROMATOGRAPHY_SUMMARY        	3.0 × 5 mm, 2.7 µm) were employed and held at 50 °C. The chromatography
CH:CHROMATOGRAPHY_SUMMARY        	gradient was initiated at 70% of B at 0–1 min, increased to 86% at 3.5–10
CH:CHROMATOGRAPHY_SUMMARY        	min, and reached 100% B at 11–17 min. The initial conditions were restored by
CH:CHROMATOGRAPHY_SUMMARY        	minute 17, followed by a 2-minute re-equilibration, resulting in a total running

CH:CHROMATOGRAPHY_SUMMARY        	methanol:isopropanol (50:50, v/v) mixture with a 15-second wash time and an
CH:CHROMATOGRAPHY_SUMMARY        	aqueous:organic phases (30:70, v/v) mixture to achieve the initial conditions.
